Request a Certificate of Birth Resulting in Stillbirth

The Office of Vital Records issues Certificates of Birth Resulting in Stillbirth for fetal deaths of 20 weeks or more gestation. Authority: Minnesota Statutes, section 144.222.

Only the Office of Vital Records (OVR) sells Certificates of Birth Resulting in Stillbirth.

To request and buy a Certificate of Birth Resulting in Stillbirth, you must:

  • Be a parent named on the fetal death report
  • Fill in the Certificate of Birth Resulting in Stillbirth (PDF) application
  • Show lawful identification and, sign and date the application in front of a notary public
  • Send your application and fees to the Office of Vital Records

OVR does not have a service desk; we accept application forms by fax or mail (postal service). We return request forms that are incomplete or not signed in front of a notary public. If your fees do not cover the services you asked for, OVR will return your request.

Credit for Parents of Stillborn Children

Parents who experience the stillbirth of a child in Minnesota may be eligible for a refundable credit of $2,000. To fill out the tax form for the credit, parents need a Certificate of Birth Resulting in Stillbirth.
